Please follow the instructions below for registration. The following will need to be submitted with the completed registration form, and non-refundable $150 per student registration fee, before registration is complete: Copy of birth certificate Copy of Baptismal Certificate (if Catholic) Copy of report card (entering grades 2 – 8) Payment option form Applicants will be admitted in this order: 1. Children whose siblings are presently enrolled 2. Children of St. Olaf Parish 3. Children from other Catholic parishes 4. Children of non – Catholic families You may mail in your registration materials. We will inform you of your acceptance status in March. If we are unable to accept your child, your registration fee will be refunded to you. Once accepted, the following items need to be submitted in order to complete the registration process: Pink immunization form Blue emergency card Signed FACTS tuition agreement Fair Share Tuition application is available at www.factstuitionaid.com. You may apply any time after registration materials and fee are submitted.
